Driving Skills Testing

What are the requirements to take a driving skills test?

  • Must have held a Learner Permit at least 12 months or until 18th birthday
  • Minors must be accompanied by a parent (no exceptions)
  • 18 or older must be accompanied by a licensed driver age 21 or older
  • Payment of $6.25 non-refundable service fee
  • Payment of any required re-exam fees

What do I need to renew my registration/license plate in person?

If you wish to process your Registration/License Plate you MUST have:

  • A valid Florida Driver License or ID card on file with the Florida Department of Highway Safety and Motor Vehicle (DHSMV)
  • Florida Insurance on file with DHSMV.
  • A Florida residential address.
  • Your last Florida Registration issued or Florida Driver License/ID card (in hand).

What do I need to renew my registration/license plate on-line?

If you wish to process your Registration/License Plate on-line you MUST have:

  • A valid Florida Driver License or ID card on file with the Florida Department of Highway Safety and Motor Vehicle (DHSMV)
  • Florida Insurance on file with DHSMV.
  • A Florida residential address.
  • Your last Florida Registration issued or Florida Driver License/ID card (in hand).

    The last registration or Driver License/ID card is needed because an on-line renewal customer must provide either the transaction number or inventory number which appear on those documents for entry into the "GoRenew" system.

  • Date of Birth and Social Security are also needed and must match what is on file with DHSMV.
  • Customers can not call to get a correct license plate number, transaction number, DL/ID Number or form number over the telephone.

Driver License/Identification Card

What documents are needed for the issuance of a Real-ID compliant Florida DL or ID?

Please bring the following documents for issuance of a Real ID-compliant Florida Driver License or Identification Card. Be prepared to have your photo taken and your vision checked. If corrected lenses are required, please have them with you.

  • Certified U.S. birth certificate or valid U.S. passport in current name
    • If customer’s name has changed since birth, we need certified proof of legal name change(s) (certified marriage certificate, certified court order, etc). Documents must link birth record to current name.
  • Social Security card or acceptable proof of number
    • Social Security name change must be made at least 24 hours prior to visiting our office
  • Florida residential address requirements (no post office box)
    • Two proofs of current Florida residential address in your name
    • If using two proofs of current Florida residential address in another person’s name, you must prove a family connection with a marriage certificate, birth certificate, or similar documentation
    • If no family connection can be established with documents presented, a Shared Residence Certification of Address Form must be submitted along with two proofs of residential address from another person. Follow the instructions on the form for acceptable residential proof.
  • Previous license if not lost or destroyed
  • See section 10 of the Florida Driver Handbook for additional license requirements
  • Military members and veterans should present a copy of their DD-214, which shows their honorable discharge from active duty, to receive a veteran designation.

Please be aware the documents must be certified originals, photocopies of the documents cannot be acceptedHospital birth certificates are not considered a certified document and will not be accepted.

How can I tell if my driver license or ID card is Real-ID compliant?

REAL ID-compliant cards have a gold star on them. If you are changing the name on a REAL ID compliant card, you will need to provide name change documents (e.g. marriage certificate, divorce decree). If you are visiting an office for a driver license or identification card transaction, bring proof of address with you even if you have not moved since your last transaction.

What documents are needed for US citizens to be Real-ID compliant?

U.S. citizens renewing, replacing or applying for a new driver license or ID card in person must submit the following documents before a REAL-ID compliant driver license or ID card can be issued. Please note that the name assigned to the Social Security Number (SSN) must match the name that will appear on the Florida driver license or ID card. Customers who have recently changed their name should update their records with the Social Security Administration before applying for a driver license or ID card.

1. Primary Identification

An original of one of the following documents with complete name:

  • U.S. birth certificate, including some U.S. territories and District of Columbia (birth certificates from Puerto Rico must have an issue date after July 1, 2010);
  • Valid U.S. passport or passport card;
  • Consular Report of Birth Abroad;
  • Certificate of Naturalization, form N-550 or form N-570; or
  • Certificate of Citizenship, form N-560 or form N-561.

NOTES

  • Only a birth certificate issued by a county health department or the CDC Bureau of Vital Statistics will be accepted. Hospital birth certificates are not considered a certified document and will not be accepted.
  • Please come prepared to present one of the above listed identification documents as proof of citizenship or legal presence.
  • When applicable, marriage certificates, court orders or divorce decrees must be provided to link the name on the primary identification to the name on the driver license or ID card.
  • If a valid U.S. passport has the customer’s current name, they are not required to present additional name change documents.

2. Proof of Social Security

An original of one of the following documents with complete name and complete SSN:

  • Social Security card (with the customer’s current name);
  • W-2 form (not handwritten);
  • Pay check/stub;
  • SSA-1099; or
  • Any 1099 (not handwritten).

Customers may not use their current driver license or ID card as proof of residential address. Customers must submit TWO different documents with their residential address (printouts or faxes of these documents are acceptable). Residential address documents include, but are not limited to:

  • Deed, mortgage, monthly mortgage statement, mortgage payment booklet or residential rental/lease agreement;
  • Florida voter registration card;
  • Florida vehicle registration or title (print a duplicate registration at www.GoRenew.com);
  • Utility bill;
  • A utility hook up or work order dated within 60 days prior to the application;
  • Automobile payment booklet;
  • Selective service card;
  • Medical or health card with address listed;
  • Current homeowner insurance policy or bill;
  • Current automobile insurance policy or bill;
  • Educational institution transcript forms for the current school year;
  • Unexpired professional license issued by a government agency in the U.S.;
  • W-2 form or 1099 form;
  • Form DS2019, Certificate of Eligibility for Exchange Visitor (J-1) status;
  • A letter from a homeless shelter, transitional service provider or a half-way house verifying they receive mail for the customer. The letter must be accompanied by a Certification of Address form;

In accordance with the REAL ID Act, customers are not required to present these documents for subsequent renewals unless their information changes (e.g., name, address).

Name Changes

Customers who have legally changed their name by marriage, divorce or court order must submit the original or a certified marriage certificate, divorce decree or court order. Photocopies are not accepted. Marriage certificates must be recognized by the state of Florida. Generally, marriage certificates issued in Florida or another state in accordance with the respective laws of those states are recognized by the State of Florida. Church-issued marriage certificates are not accepted.

Note: Please allow 24 to 48 hours after changing your name with Social Security Administration before applying to change it with the Florida Department of Highway Safety and Motor Vehicles. Your name must verify with the Social Security electronic system to obtain a Florida driver license or identification card.

Certified copies of marriage certificates or divorce decrees may be obtained at the clerk of court in the county where it was originally filed.

What documents are needed for immigrants to become Real-ID compliant?

Immigrants renewing, replacing or applying for a new driver license or ID card in person must submit the following documents before a REAL ID-compliant driver license or ID card can be issued. Please note that the name assigned to the Social Security Number (SSN) must match the name that will appear on the Florida driver license or ID card. Customers who have recently changed their name should update their records with the Social Security Administration before applying for a driver license or ID card.

1. Primary Identification

An original of one of the following documents with complete name:

  • Valid alien registration receipt card (green card, form I-551);
  • I-551 stamp in passport or on I-94;
  • Immigration judge’s order, containing the customer’s A-number (admissions number),
    granting asylum;
  • I-797, containing the customer’s A-number, stating the customer has been granted asylum; or
  • I-797, or another form from the United States Citizenship and Immigration Services (USCIS), with the customer’s A-number, stating the customer’s application for refugee status is approved.

2. Proof of Social Security

An original of one of the following documents with complete name and complete SSN:

  • Social Security card (with the customer’s current name);
  • W-2 form (not handwritten);
  • Pay check/stub;
  • SSA-1099; or
  • Any 1099 (not handwritten).

1. Primary Identification

An original of one of the following documents with complete name:

  • Valid employment authorization card issued by the U.S. Department of Homeland Security (DHS) form I-688B or I-766;
  • Non-expired proof of non-immigrant classification provided by DHS form I-94 with required supporting attachment(s). If a customer is unsure of the required documentation, they should bring all United States Citizenship and Immigration Services (USCIS) documentation with them.
    I-94s must be accompanied by a valid passport with a date of entry stamp. Certain classifications require additional documentation. Some examples are:

     

    • F-1 and M-1 classification must be accompanied by an I-20;
    • J-1 or J-2 designation must be accompanied by a DS2019;
    • Refugee, asylee and parolee classifications must be accompanied by additional documentation;
  • I-571 travel document/refugee travel permit;
  • I-512 parole letter accepted; or
  • Immigration judge’s order granting asylum or cancellation of removal.

NOTES

  • Documents must be valid for more than 30 days from the date of issuance.
  • Non-U.S. citizens applying for an original driver license will be issued a 60-day temporary paper permit without a photo and a receipt. Non-U.S. citizens applying for an ID card will be issued a receipt. Upon identity and legal status verification, a driver license or ID card will be issued within 60 days and mailed to the address on the driver record. The driver license or ID card will be issued for the period of time specified on the USCIS document, up to a maximum of four years.
  • If lawful status or identification is not verified, a denial of issuance letter will be mailed to the customer.

What do I need to bring for original or out-of-state titles?

  1. Proof of Identification:
    • A Florida driver license or identification card
    • An out of state driver license or identification card with photo
    • A US passport
    • A Canadian driver license, identification card or passport
    • A driver license or identification card from any U.S. Territory (American Samoa, Guam, Marianas, Puerto Rico and US Virgin Islands)
    • An out of country passport

  1. Proof of ownership:
    • MCO (Manufacturer’s Certificate of Origin) – initial ownership documentation on a new motor vehicle, mobile home or vessel
    • Out of state title on a used motor vehicle, mobile home or vessel. If the motor vehicle, mobile home or vessel was not required to be titled, then proof of ownership is a dealer’s bill of sale or the original registration from that state and a bill of sale from the seller to the purchaser.
  1. Form 82042, Vehicle Identification Number (VIN) and Odometer Verification, accurately completed by the purchaser, with the motor vehicle identification number verified by one of the following:
    • Law enforcement office from any state
    • Licensed Florida or out of state motor vehicle dealer. (If the verification is done by an out of state dealer, the verification must be submitted on dealer’s letterhead stationery.)
    • Florida DMS Compliance Examiner, DMS or tax collector employees. (The Tax Collector’s office can only complete the verification if weather conditions permit.)
  1. Form 82040, Application for Certificate of Title with/without Registration accurately completed. (A Form 82041 may be signed in lieu of the 82040 if all persons are present for the transfer). If a lien is to be recorded, the lien information must be provided in the appropriate space of the Form 82040. This includes the lienholder’s name, address, and federal employer’s identification FEID) number. When an individual is the lien holder, their name, address and driver license number is required. Should the individual lienholder be from another state, a copy of their identification is needed.

  1. A Bill of Sale showing the purchase price, trade in allowance and sales tax paid is needed. Payment of sales tax is due at the time of application unless there is a reason for sales tax exemption. Leon County collects 6% sales tax on the total price and an additional 1.5% on the first $5,000.

  1. Title Fees:
    • Original New Vehicle Title Fee - $77.75
    • Original Used Out of State Vehicle Title Fee - $85.75
    • Original New Vessel title Fee - $5.75
    • Original Used Out of State Vessel - $9.75

What do I need to bring to transfer a Florida title?

Please bring the following documents for Florida title transfers:

  1. Proof of Identification:
    • A Florida driver license or identification card
    • An out of state driver license or identification card with phot
    • A US passport
    • A Canadian driver license, identification card or passport
    • A driver license or identification card from any U.S. Territory (American Samoa, Guam, Marianas, Puerto Rico and US Virgin Islands)
    • An out of country passport
  1. Form 82040, Application for Certificate of Title with/without Registration accurately completed. (A Form 82041 may be signed in lieu of the 82040 if all persons are present for the transfer.)
  1. If a lien is to be recorded, the lien information must be provided in the appropriate space of the Form 82040. This includes the lienholder’s name, address, and federal employer's identification (FEID) number. When an individual is the lien holder, their name, address and driver license number is required. Should the individual lienholder be from another state, a copy of their identification would be needed.
  1. An original lien satisfaction, (Form82260) is required for any lien against the previous owner of a motor vehicle, vessel, or mobile home, which is shown on the DMS database but is not satisfied on the front of the Florida title.
  1. Payment of sales tax is due at the time of transfer unless there is a reason for sales tax exemption. Leon County collects 6% sales tax on the total price and an additional 1.5% on the first $5,000.
  1. Proof of current Florida insurance must be presented in the form of an automobile policy, ID card, or binder. If the vehicle will not be driven on the highways of Florida, the applicant must complete a non-use affidavit.
  1. The Florida license plate number to transfer or a new plate may be purchased for the motor vehicle. However, a mobile home registration is automatically issued when application for title is made.

  1. Title Fees:
    • Transfer Title Fees - $75.75
    • Lien Fee - $2.00 (per Lien if applicable)
    • Penalty Fee - $20.00 - is assessed on a title not transferred within 30 days of the date of purchase
    • Fast Title Fee - $10.00 (title issued at time of application; fee is in addition to the title fee, and some restrictions apply)
    • Shipping and Handling Fee - $2.50 - for each paper title printed and mailed

What do I need to bring to obtain a duplicate Florida title?

The owner of the vehicle, vessel or mobile home or their appointed Power of Attorney (POA) Form 82053, must bring the following documents into the Tax Collector’s Office:

  1. Current Proof of Identification:
    • A Florida driver license or identification card.
    • An out of state driver license or identification card with photo.
    • A US passport.
    • A Canadian driver license, identification card or passport.
    • A driver license or identification card from any U.S. Territory (American Samoa, Guam, Marianas, Puerto Rico and US Virgin Islands).
    • An out of country passport.
  2. Form 82101, Application for Duplicate or Lost In Transit/Reassignment for a Motor Vehicle, Mobile Home or Vessel Title Certificate accurately completed. If there is more than one vehicle owner and the title is connected by "and", all parties will have to sign the application.
  3. When the address on the Form 82101*, Application for Duplicate or Lost In Transit/Reassignment for a Motor Vehicle, Mobile Home or Vessel Title Certificate is different from the address on DMS records, submit a copy of one of the following proofs of change of address:
    • The motor vehicle owner’s photo ID (Driver License, Florida ID, Business credential, etc.)
    • Paid receipt for telephone or utilities.
    • Paid contract or connect order for utilities
    • Proof of homestead exemption
    • Rental or lease contract agreement
    • Current year vehicle registration certificate
    • Homeowner's or renter's insurance policy
  4. If the lien has been satisfied, an original or certified copy of a  lien satisfaction, Form 82260 must be submitted with the duplicate title application.
  5. A current odometer reading is required on a duplicate or lost in transit application when applied for by the owner. If the vehicle is 10 years old or older, the vehicle is exempt from all odometer requirements. Write the current odometer reading or "exempt" in the odometer boxes.

Concealed Weapons

What do I need to bring to my concealed weapons appointment?

  1. Picture identification (IE: a valid driver license or state issued ID card)
  2. A copy of an approved firearms training document (IE: certificate of completion or proof of competency from a firearms safety course/training class conducted by a state-certified instructor an instructor certified by the National Rifle Association)
  3. Other documents may be required: See Eligibility Requirements

What do I need to bring to obtain a birth certificate?

  • Valid Identification must be provided by all eligible applicants to obtain a duplicate Birth Certificate.
  • Child's full name presented as it appears on the official birth record; including, any subsequent name changes.
  • City and County where the child was born must be indicated.
  • Mother's full name presented (including, mother's maiden name).
  • Father's full name presented (if included on the official birth record).

What do I need to bring to obtain a permanent disabled parking permit?

Permanent parking permits are issued for up to four years and expire on the customer’s birthday. Renewals are for a full four years.

The customer must have:

  • A valid Florida Driver License or Identification Card
  • Application HSMV 83039 signed by the customer and the certifying authority within the last 12 months
  • There is no charge for a permanent parking permit.

Permanent parking permits may be renewed by mail (as long as the applicant has submitted/submits the form HSMV 83039 completed and signed by the certifying authority within the last 12 months), or in person in our office.

What do I need to bring to obtain a temporary disabled parking permit?

Temporary parking permits are issued for a period up to six months.

The customer must have:

  • A valid Florida Driver License or Identification Card
  • Application HSMV 83039 signed by the customer and the certifying authority
  • The cost for the temporary parking permit is $15.
  • If a second temporary parking permit is issued to the customer within 12 months there will be no charge.

Temporary parking permits may not be renewed on-line or by mail; you must appear in the office with your completed HSMV 83039 signed by the certifying authority.
